Log in or Create an Account to your Patient Portal to complete documents related to consent of treatment, questionnaires, screening tools or history, needed prior to treatment.
Use the Patient Portal to communicate with your provider, upload or fax medical records.
Check your internet and connection prior to the appointment. You will get another reminder proceeding your appointment.
Your appointment will be conducted through secure, HIPPA compliant video conferencing accessed from the Patient Portal.
Payment and Cancellation
The credit card on file to make the appointment will NOT be charged unless there is a cancellation of an appointment without 24 hours notice.
The credit card on file will only be charged if you chose a CASH only appointment. OR for a copay, designated by insurance.
