What’s Next?

  • Check your email for confirmation of appointment and links pertaining to consent, questionnaires, screening tools or background information.

  • Log in or Create an Account to your Patient Portal to complete documents related to consent of treatment, questionnaires, screening tools or history, needed prior to treatment.

  • Use the Patient Portal to communicate with your provider, upload or fax medical records.

  • Check your internet and connection prior to the appointment. You will get another reminder proceeding your appointment.

  • Your appointment will be conducted through secure, HIPPA compliant video conferencing accessed from the Patient Portal.

Payment and Cancellation

  • The credit card on file to make the appointment will NOT be charged unless there is a cancellation of an appointment without 24 hours notice.

  • The credit card on file will only be charged if you chose a CASH only appointment. OR for a copay, designated by insurance.
